Near Miss Reporting Form

Reporting a Near Miss is a very simple process.  This is more of a summary report indicating an unsafe act, condition, and equipment use or malfunction.  This summary data can then be used to facilitate training and/or the elimination of the potential near miss so that it does not turn into a Incident or accident.
