Laboratory Registration Process
This is an example of a simple lab registration process that allows the responsible P.I./Lab Manager to register himself/herself along with the room/labs and personnel that will work within their lab group. In addition, there is a hazard assessment form that identifies potential hazards that this group will be required to deal with on a day-to-day basis. This helps with the identification of necessary Training, PPE, Medical Surveillance, Lab Placarding, and additional questionnaires that are required. This registration can be updated once submitted and approved recording any changes identified by the P.I./Lab Manager without having to re-key that data from the form.
